This QRG will walk you through entering in a 2728.

Adding a New 2728

1. Click Registration > Pre-Dialysis Information (2728).

2. Add New.

3. Fill out the form.

• Note: Required fields are: 2728 type, Date Regular Dialysis Began, Cause of Renal Failure, and Pre-Dialysis Weight.

4. When finished, click Add.

Updating an Existing 2728

1. Click Registration > Pre-Dialysis Information (2728).

• Note: Ensure that current patient is the correct one to view or modify. If not, click Change and select appropriate patient.

2. Click on the 2728 type that needs to be updated.

3. Update information > Submit.
